Acceptance of Terms
By accessing or using the services provided by Heal & Conquer Counseling Services, you agree to be bound by these terms and conditions and my privacy policy. If you do not agree with any part of these terms, you may not access or use our services.
Services Provided
Heal & Conquer Counseling Services offers mental health services, including but not limited to counseling, therapy, assessments, coaching, and other related services. The specifics of the services provided will be discussed and agreed upon between you (the client) and Heal & Conquer Counseling Services (the provider/owner).
Patient Responsibilities
Provide accurate and complete information about your health history, current symptoms, and other relevant details.
Attend scheduled appointments or provide sufficient notice for cancellations or rescheduling.
Follow the treatment plans and recommendations provided by your mental health provider.
Inform your provider of any changes in your health status or medication regimen.
Payment and Billing
Payment for services is due at the time of service unless otherwise agreed upon.
We accept: Visa, MasterCard, Discover, American Express, Flexible Spending Account (FSA) and Venmo.
Insurance coverage and billing procedures will be discussed and agreed upon separately.
Confidentiality
Your privacy and confidentiality are important to us. We adhere to all legal and ethical standards regarding the confidentiality of your personal and health information. Please refer to our privacy policy for more details.
Communication
By using our services, you consent to receive communications from us electronically or via other agreed upon means.
We may communicate with you regarding appointments, resources, reminders, billing, treatment updates, and other administrative matters.
Termination of Services
Either party may terminate the therapeutic relationship at any time with reasonable notice. If you wish to terminate services, please inform your mental health provider directly.
Limitation of Liability
Heal & Conquer Counseling Services and its providers are not liable for any direct, indirect, incidental, consequential, or punitive damages arising from your use of our services or inability to access our services.
Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the state and local laws, without regard to its conflict of law principles.
Changes to Terms and Conditions
We reserve the right to update or modify these Terms and Conditions at any time without prior notice. Any changes will be effective immediately upon posting on our website or notifying you directly.
